Step 1 — Practice Information
Basic identity details that appear throughout the portal
Welcome to Active SOS Practice Setup. This form takes about 10 minutes to complete. You can fill it out during your onboarding call or your account manager can send you the link ahead of time. At the end you'll receive a confirmation email and a downloadable setup file for your records.
Practice Identity
The full legal or operating name of the practice as it should appear in the portal and billing documentation
10-digit National Provider Identifier — used for billing documentation
For future integration planning

Practice Address
📞
Step 2 — Contacts & Phone Numbers
How patients and Active SOS will reach the practice
Alert Notification Number: This is the number that receives an immediate text when a patient sends an SOS alert. This is typically the doctor's cell phone or the office urgent/after-hours line. Make sure it's monitored during practice hours.
Practice Phone Numbers
Primary voice line — displayed in the portal and used for patient communication
If your practice already uses a texting platform (e.g. Klara, Spruce, RingCentral). Leave blank if not applicable — Active SOS will assign you a dedicated SMS number.
Receives immediate SMS when any SOS alert fires. Confirm this is monitored 24/7 or during all practice hours.
If different from the alert number — used for escalation routing in a future phase

Care Coordinator — Portal Admin
This person will receive portal login credentials. They will be the primary user monitoring the patient message queue and managing RTM billing documentation.
Person who will monitor the message queue daily
A portal login and set-password link will be sent to this address
For Active SOS support to reach the coordinator directly

Step 3 — Alert Configuration
Choose your specialty defaults, then customize
How alert setup works: Your specialty defaults are pre-loaded into the portal during onboarding. After your first login go to Practice Settings → Alert Configuration to review, toggle, reorder, or add custom alerts at any time.
1. Select Your Specialty Defaults

This determines which alert list is pre-loaded when your account is created.

3. Custom Alerts to Add (optional)
💳
Step 4 — RTM Billing Defaults
Set time credited per care activity — used for CPT code tracking
How RTM time tracking works: Every time your care coordinator reviews a message, escalates an alert, or sends a reply, the portal automatically credits the minutes you set below toward the patient's monthly RTM total. The first 20-minute threshold qualifies for CPT 98980 / 99457. Each additional 20 minutes adds 98981 / 99458. These defaults can be adjusted at any time in Practice Settings.
📋 Message Reviewed
Time credited when coordinator clicks "Mark Reviewed" on a patient message
minutes  •  current: 3 min
🚨 Alert Escalated
Time credited when coordinator escalates an SOS or SAFE alert to the physician
minutes  •  current: 5 min
💬 Message Reply Sent
Time credited when coordinator sends a reply to a patient message
minutes  •  current: 3 min
Set realistic defaults. These should reflect actual average time spent per activity. CMS audits RTM billing based on documented time. If your coordinator typically spends 2 minutes reviewing a routine check-in, set Message Reviewed to 2 min. You can adjust in the portal at any time.
Step 5 — Review & Submit
Confirm your details, then submit to Active SOS
