Basic identity details that appear throughout the portal
Welcome to Active SOS Practice Setup. This form takes about 10 minutes to complete.
You can fill it out during your onboarding call or your account manager can send you the link ahead of time.
At the end you'll receive a confirmation email and a downloadable setup file for your records.

How patients and Active SOS will reach the practice
📱 Active SOS Dedicated SMS Number (Twilio)
ASSIGNED BY ACTIVE SOS
This is the Twilio number assigned to this practice in the Active SOS system.
All patient text messages arrive on this number and are routed to the Connect portal via SMS webhook.
One dedicated number per practice. Leave blank if not yet assigned — your account manager provisions this before go-live.
Alert Notification Number: This is the number that receives an immediate text when a patient sends an SOS alert.
This is typically the doctor's cell phone or the office urgent/after-hours line. Make sure it's monitored during practice hours.
Practice Phone Numbers
Primary voice line — displayed in the portal and used for patient communication
If your practice already uses a separate texting platform (e.g. Klara, Spruce, RingCentral). This is different from your Active SOS/Twilio number above — leave blank if not applicable.
Receives immediate SMS when any SOS alert fires. Confirm this is monitored 24/7 or during all practice hours.
If different from the alert number — used for escalation routing in a future phase
Portal Users
These users receive Connect portal login credentials.
The Primary Admin (required) is the main user monitoring the message queue and RTM billing.
You can list up to 10 users total (1 primary admin + 9 additional) below — add physicians,
coordinators, or billing staff you want set up ahead of time. Leave any rows blank if you don't need them yet;
blank rows are ignored on submit. Need more than 10? Use the "Add Another Portal User" button below the table.

Step 3 — Alert Configuration
Choose your specialty defaults, then customize
How alert setup works: Your specialty defaults are pre-loaded into the portal during onboarding.
After your first login go to Practice Settings → Alert Configuration to review, toggle, reorder, or add custom alerts at any time.

Step 4 — RTM Billing Defaults
Set time credited per care activity — used for CPT code tracking
How RTM time tracking works: Every time your care coordinator reviews a message, escalates an alert, or sends a reply,
the portal automatically credits the minutes you set below toward the patient's monthly RTM total.
The first 20-minute threshold qualifies for CPT 98980 / 99457. Each additional 20 minutes adds 98981 / 99458.
These defaults can be adjusted at any time in Practice Settings.

Set realistic defaults. These should reflect actual average time spent per activity.
CMS audits RTM billing based on documented time. If your coordinator typically spends 2 minutes
reviewing a routine check-in, set Message Reviewed to 2 min. You can adjust in the portal at any time.

What happens when you submit: Your account manager at Active SOS is notified immediately.
They will provision your dedicated Twilio SMS number (if not yet assigned), configure the SMS webhook,
create portal accounts for all listed users, and send login credentials within 1 business day.
Your onboarding call will walk through Alert Configuration together in the portal.
